Job Description:
Registry- Days
Reporting to the Clinical Manager of Home Care provides diagnostic and therapeutic intervention for speech, language, voice, swallowing and fluency disorders. Functions as an integral member of the rehabilitation team attending conferences and in-servicing team members as to the effects of communication and swallowing disorders. Provides diagnostic and therapeutic services to home health patients and to other designated locations.
This position supports the north team consisting of north of Roosevelt Road to the Illinois/Wisconsin border; east from the lake to the northwest suburbs.
JOB SPECIFICATIONS:
A Master’s Degree in Speech and Language Pathology and Certificate of Clinical Competence.
State of Illinois licensure.
One (1) to two (2) years of experience within a clinical/hospital setting preferred.
Current Illinois driver license and current auto liability insurance
Valid CPR card.
The ability to function as part of a multidisciplinary treatment team, share professional knowledge, to develop integrated treatment plans and gain peer support for the patient’s speech and pathology regimen.
The ability to respond flexibly to the needs of the patient and the needs of Home Care management and staff, the ability to adopt programs and procedures to accommodate the situation
The creativity needed to develop treatment approaches to address individual specific patient problems.
Strong verbal and written skills to communicate program needs, assess program activity, conduct therapy, and interface with patients and family.
