Job Description:

~Alexian Rehabilitation Hospital is the unique collaboration between two distinguished institutions-Alexian Brothers Health System and the Rehabilitation Institute of Chicago.
 
This opportunity will be stationed within the Inpatient Rehab unit and float to the partnership areas of care to include: Med/Surg, Outpatient, Day Rehab, and Outpatient locations in ElK Grove Village and Schaumburg, IL. as required.

PRIMARY PURPOSE:     
 
Under the general direction of the manager of physical therapy, performs evaluations and provides treatment for inpatients and outpatients within the scope and professional ethics of the physical therapy profession. Assists manager in department planning and organization.
 
 
JOB FUNCTIONS: 
 
Evaluates the patient’s physical and functional status.
Establishes short and long term goals and formulates a treatment plan consistent with goals, age and developmental level.
Demonstrates competency in providing patient treatment consistent with patient’s physical and functional status including patient’s age and developmental status.
Documents patient evaluation and treatment progress according to department policy.
 
Demonstrates the ability to develop an effective plan and treatment goal that best facilitate optimal patient outcomes.
 
Follow departmental policies and procedures related to written documentation and demonstrate high quality of content and timeliness of completion.
 
Effectively communicates with others such as physiatrists and team members regarding patients' progress and treatment program recommendations and/or with family including education/training.
 
Meets productivity expectations, demonstrates the organization prioritization and time management skills necessary to accomplish the responsibilities and special assignments.
 
Displays constructive and motivational approach with others and the qualities that make a team player and positively represent the Medical Center and the PT Department.
 
 
Shares in the Medical Center's Vision, demonstrates its Values, supports its Philosophy, and is sensitive to its Mission.
 
As needed recommends quality and/or process improvement initiatives in order to more effectively and efficiently perform the Job Functions of this job.
 
Uses tact and sensitivity and communicates effectively with patients, visitors, co-workers and other hospital personnel.
 
Demonstrates awareness of patients’ needs as a total person (physical, spiritual, emotional, intellectual, cultural, etc.), providing care consistent with patients age and developmental level.
 
Performs all other Job Functions related to this Job.
 
 

Job Requirements:

~MINIMAL JOB REQUIREMENTS:    (necessary to adequately perform the Job Functions)
Requires a Bachelor of Science Degree with a certificate or degree in physical therapy, or Master’s Degree in physical therapy from APTA approved program.
Must be licensed in Illinois or eligible for licensure in accordance with Illinois state law.
Must be able to lift 50 lbs.
Must be able to participate in physical management of patients, including both physical restraint and the application of mechanical restraint when necessary.
Must be able to assist in the physical care of patients, including but not limited to lifting/turning of patients confined to bed and/or wheel chair.
 
A collaborative persona- ability to work alongside a multidisciplinary team proactively.
