Job Description:

~The Physical Therapist develops and implements individual physical therapy programs designed to improve and maximize patient mobility and independent functioning within their physical limitations/disabilities.  Provides treatment on a home health basis in the home or designated locations.
 
Specific Responsibilities:
 
1. Evaluates the patient’s functional abilities/disabilities, range of motion and muscle testing through assessment and testing procedures. Establishes the patient’s functional base-line measurement to be used throughout treatment as a gauge of progress and physical improvement.
2. In accordance with patient evaluation, establishes treatment plans which specify treatment goals and the treatment modalities that will assist the patient to regain function, increase strength and decrease pain. Modifies the treatment plans as patient’s condition changes.
3. Performs physical, electrical, mechanical and heat treatments (facilitation), strengthening and mobilization exercises, gait training, prosthetic training, massage, manual traction, ultrasound, hot pack application, Jobst RX, transcutaneous electrical neural stimulation, pulsed lavage, iontophoresis. Trains the patient with DL/adaptive equipment and in the appropriate home exercise program.
4. Supervises treatments provided by the physical therapist assistants, reviews treatment plans and patient progress with fellow treatment team members. Instructs and supervises ancillary personnel in proper technique/treatment procedures.
5. As necessary, discusses treatment plans and patient progress with nurses, physicians, family members and other disciplines regarding discharge planning including but not limited to the regimen of home treatment, the patient’s physical level of independence and the level of nursing care needed.
6. Instructs and trains the patient and the patients’ family, or other responsible person, in home exercises, transfers and ADLS as appropriate.
7. Documents the patient’s treatment and progress in the patient’s chart (or outpatient records) indicating the condition in relation to their established medical goals. Completes Medicare records and progress notes.  Co-signs the physical therapists assistants’ documentation.
8. Provides in-services concerning physical therapy to hospital staff and/or the community in physical therapy.
9. Keeps abreast of developments in the physical therapy field and incorporates the approved developments in treatment and evaluation plans.
10. Assists in Quality Improvement monitoring and analysis.
11. Provides patient and family with information, support and encouragement which may    help them to adopt attitudes and practices that promote health and reduce anxiety and tension. Assists the family/patient in accepting the appropriate medical, hospital, nursing home, welfare or other necessary care. Interprets the extent/limitations of available services, arranges referrals and communicates pertinent patient/family information to the appropriate agency upon request.
12. Remains current of developments in the field of physical therapy by reading pertinent literature. Incorporates the same into physical therapy practice where applicable and appropriate.
13. Evaluates the environmental safety and provides an environment that is safe based on the age and developmental status of the patient.

Job Requirements:


1. Current licensure as a Physical Therapist in the state of Illinois.
2. Current Illinois driver license and auto liability insurance.
3. Current CPR certification.
4. A Bachelor’s Degree or certificate in Physical Therapy from an American Physical Therapy Association approved school which would provide an understanding of anatomy, physiology, pathology and physics necessary to assess the patients’ condition and develop treatment plans and goals.
5. The physical ability to lift/move/ambulate patients.
