SCOPE OF POSITION

This position will demonstrate competency in the care of the: Pediatric, Adolescent, Adult, Geriatric patient population.
The Physical Therapist (PT) as a member of the Home Health team has the responsibility to provide treatment to patients as prescribed by the PT. The PT should carry out the plan of treatment established by the PT, plan for discharge, modifying and adjusting the treatment plan upon instruction from the PT. The PT will have good documentation skills and succinctly document patient's status and develop measurable skills. The PT works in conjunction with other team professionals, patients and their families to provide patient care and education as well as participates in in-services programs.
The PT should have the ability to provide services to pediatrics, adolescents, adult and geriatric patients; must be able to assess and interpret data about patient's age-specific needs and provide appropriate care to patient group.
The PT will also carry out other departmental duties as assigned. The PT must have the ability to work cooperatively with physicians, other staff members, patients and their families with dignity, courtesy, and respect. The PT must show initiative and good judgment, be outgoing and flexible to adjust to all possible situations and have flexibility in hours to meet the needs of HomeCare and Hospice patients and to participate in weekend rotations as needed. The PT must pay close attention to detail and provide superior levels of care. The PT should be committed to continuing education, upgrading his/her skills, and be willing to cross train in other areas as necessary.
This position may be required to cross train within the agency. Responsibilities in secondary position may require access to medical records and other computer access exceeding routine computer usage. Access to patient information may be necessary to fulfill the requirements of this role; therefore, adherence to MSHA privacy and security policies and practices is a must.

It is vital that an individual in this position be capable of good communication skills. It is of the utmost importance that written communication is legible.

Job duties of this position may require access to protected patient information (PHI). The team member will be accountable for appropriate use of the record and compliance with all confidentiality and security policy and procedures related to use, access, and disclosure of PHI.
This position reports to the Manager of Therapy Services and supervises the PTAs who are giving care to the patients assigned to the PT. May supervise CNA's when skilled nursing is not included in patient's plan of care.
